Position Overview:

• Employment Type: Full-Time, Permanent
• Locations: Multiple locations (based on availability)
• Specialty: Geriatric Primary Care / Home-Based Primary Care
• Patient Population: Geriatric / Older Adults
• Patient Ratio: Average of 5-9 patients a day
• Setting: Home-Based Primary Care
• Base Salary Range: $155k - $165k annually

Schedule:

• Monday–Friday | No weekends or evenings
• No holidays or hospital rounds
• Minimal call responsibilities

Responsibilities:

• Provide personalized, home-based primary care to high-risk geriatric patients, focusing on their individual healthcare needs rather than patient volume.
• Assess, diagnose, and manage chronic conditions while developing individualized treatment plans to improve patient outcomes.
• Coordinate comprehensive care for patients by leveraging experience in primary care, geriatrics, home health, or hospice, ensuring compassionate, patient-centered treatment.

Requirements:

• A licensed Nurse Practitioner or Physician Assistant in the state where the practice is located.
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record
