What You'll Do

Travel to assigned Elara Caring locations to provide home health physical therapy.
Evaluate patients, develop treatment plans, and help them regain mobility and independence.
Collaborate with local clinical teams to deliver exceptional patient care.
Document patient progress and support continuity of care.

What You'll Need

Degree from an accredited Physical Therapy program.
Active PT license or ability to obtain licensure in assigned states.
At least one year of clinical experience.
Willingness and ability to travel 100% to assigned locations.
Valid driver's license and strong communication skills.
