Travel Physical Therapist (PT) – Skilled Nursing Facility

We are seeking a Travel Physical Therapist for a Skilled Nursing Facility contract in Columbus, Ohio. This opportunity is a great fit for a PT who enjoys post-acute rehabilitation and helping adult and geriatric patients improve mobility, strength, balance, safety, and functional independence.

This position offers 35–40 guaranteed hours per week and is new-grad friendly. Both local and travel candidates may be considered.

Responsibilities

• Perform physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient needs and functional goals.
• Provide therapeutic exercise, gait training, balance training, transfer training, and mobility interventions.
• Help patients improve strength, endurance, safety, and overall independence.
• Educate patients and caregivers on mobility strategies and home exercise programs.
• Collaborate with nursing staff, OT, SLP, and the interdisciplinary rehab team.
• Complete accurate and timely clinical documentation.

Requirements

• Ohio PT license required.
• BLS required.
• SNF or post-acute rehabilitation experience preferred.
• New graduates may be considered.
• Strong communication and teamwork skills preferred.
