RN – Case Manager

Eureka, CA

Start Date: 11/16/2026
Contract: 26 Weeks
Shift: Days | 5x8 | 8:00 AM – 4:30 PM
Weekend: Every Other Weekend

Requirements:

Minimum 2 years of acute hospital Case Management experience.
Must have recent acute care Case Management experience within the last year.
Experience in care coordination and discharge planning.
Epic experience required.
Previous travel experience required; first-time travelers may be screened with Clinical approval.
Experience with HMOs, IPAs, etc.
Experience with Advance Directives, POLST, and protective services processes.
Experience with admission criteria, utilization review, appeals/denials, benefits eligibility, and DME needs assessment.
Acute hospital experience across ED, ICU, MS, Pediatrics, and SDU/PCU/IMC/Observation preferred.
