💪 What You'll Do

• Evaluate and treat patients in their homes, providing individualized physical therapy services.
• Develop personalized treatment plans focused on improving mobility, strength, balance, safety, and functional independence.
• Assist patients recovering from hospitalization, surgery, illness, injury, or changes in functional status.
• Provide therapeutic exercise, gait training, transfer training, balance activities, and fall-prevention interventions.
• Assess patients' home environments and recommend strategies to improve safety and independence.
• Educate patients and caregivers on home exercise programs, mobility techniques, and injury prevention.
• Monitor patient progress and adjust treatment plans to support functional goals.
• Collaborate with physicians, nurses, occupational therapists, and other home health professionals.
• Complete accurate and timely clinical documentation in accordance with home health requirements.
• Travel between patient homes within the assigned service area.

📋 Requirements

• Active Oregon Physical Therapist license or eligibility to obtain.
• Previous Home Health experience preferred.
• Experience with OASIS assessments and home health documentation preferred.
• Current BLS certification.
• Valid driver's license and reliable transportation.
• Strong clinical assessment, communication, and patient education skills.
• Ability to work independently and manage a community-based patient caseload.
• Excellent time-management and organizational skills.
