Required Qualifications
Minimum 2 years of acute-care medical Case Manager experience performing care coordination and discharge planning
Epic experience required
Acute hospital experience
Experience with case management/utilization review, medical necessity guidelines, discharge planning, care coordination, disease management, needs
assessments, and DME orders
Experience with HMOs, IPAs, and computerized documentation
Experience with ICU, MS, and SDU/PCU/IMC/Observation patients
Travel experience required; first-time travelers may be screened
California RN license required by start date; pending license accepted
