Description
Job Title: Case Manager RN 1
Location: Dallas, TX
Assignment Length: 3 Months (Possible Extension)
Schedule & Shift:
- Shift: Days (AM)
- Schedule: Monday–Friday
- Hours: 8:00 AM – 4:30/5:00 PM
Requirements:
- Active RN license required.
- Associate Degree in Nursing or graduation from an accredited nursing program required.
- Minimum 2 years of Case Management experience required.
Key Responsibilities & Skills:
- Conduct comprehensive patient/member assessments.
- Develop and implement individualized case management plans.
- Coordinate patient care across the continuum of care.
- Monitor patient progress and evaluate treatment outcomes.
- Collaborate with physicians, healthcare providers, and interdisciplinary teams.
- Identify barriers to care and coordinate appropriate resources.
- Support quality outcomes while ensuring cost-effective care.
- Maintain accurate case management documentation.
