Description
- Recent acute-care hospital case management experience, within the last 12 months.
- Epic charting experience.
- Experience with HMOs, IPAs, and similar healthcare organizations.
- Experience with advance directives, POLST, and protective services processes.
- Case management skills covering discharge planning, care coordination, utilization-related processes, admissions, and appeals/denials.
- Valid state license; compact license must be in hand at submission, per the posting.
- Travel experience.
