PAY:
Local Pay - $57 to $60/hr on W2.

Travelers Pay: 
Taxable Amount - $40/hr on W2
Lodging Stipend - $700 weekly
Meals Stipend - $567 weekly
Weekly Gross - $2,867 weekly including stipend for 40 hours.

JOB DESCRIPTION:
Position - RN-Case Manager
Location - Mattoon, IL 61938
Duration - 13 weeks contract
Day Shift - Mon - Fri, 8 am to 4:30 pm, no weekends, major holidays, or call required 

Weekly Hours - 40 hours 

Will cover all inpatient units, ED holds, and overflow units

RESPONSIBILITIES:
Assess patients' medical conditions, care needs, and discharge requirements.
Develop and coordinate individualized patient care plans.
Collaborate with physicians, nurses, social workers, and healthcare teams.
Coordinate patient admissions, transfers, and discharge planning.
Monitor patient progress and ensure appropriate utilization of healthcare resources.
Coordinate referrals to home health, rehabilitation, and other healthcare services.
Advocate for patients to ensure quality care and continuity of services.
Communicate with insurance companies regarding authorizations and medical necessity.
Monitor length of stay and identify barriers to discharge.
Educate patients and families about treatment plans and available resources.
Maintain accurate documentation and ensure compliance with healthcare regulations.
Facilitate safe and timely transitions of care.

REQUIREMENT:
Illinois RN state License.

Must have 5 years as RN, in hospital experience, with minimum of 3 years 

case management exp.

Both local and travel candidate's can apply.
