Registered Nurse (RN) – Skilled Nursing Facility (SNF)
 

We are seeking an experienced Registered Nurse (RN) to provide quality nursing care to residents in a Skilled Nursing Facility. The ideal candidate will be responsible for assessing patient conditions, administering medications and treatments, coordinating patient care, and ensuring compliance with established nursing standards and facility protocols.
 

Responsibilities:

• Provide direct nursing care to residents in accordance with individualized care plans.
• Assess, monitor, and document patients' health conditions and changes in clinical status.
• Administer medications, treatments, and prescribed therapies safely and accurately.
• Perform wound care and other skilled nursing procedures as needed.
• Monitor vital signs and promptly report any changes in patient conditions.
• Collaborate with physicians, LPNs, CNAs, and interdisciplinary healthcare teams.
• Supervise and delegate patient care responsibilities to nursing staff as appropriate.
• Maintain accurate patient records and clinical documentation.
• Ensure compliance with infection control policies, patient safety standards, and regulatory guidelines.
• Educate residents and their families regarding treatment plans and ongoing care.

Requirements:

• Active and unrestricted CA Registered Nurse (RN) license.
• Previous experience in Skilled Nursing Facility (SNF), Long-Term Care (LTC), or Rehabilitation settings
• Current BLS certification. (AHA)
• Strong clinical assessment, communication, and organizational skills.
