Minimum 2 years of acute-care Case Management/Discharge Planning experience
Acute-care hospital experience is required; behavioral health-only experience does not qualify
Epic experience
Prior travel experience; first-time travelers are not accepted
Active California RN license by start date
Experience with care coordination, utilization review, and discharge planning
Ability to arrange DME, SNF placement, and ambulance transportation independently
Experience coordinating with HMOs/IPAs
