Description
Job Responsibilities
- Performs comprehensive patient assessments and reassessments, using sound clinical judgment.
- Develops, implements, and updates individualized patient care plans.
- Monitors patient conditions, evaluates treatment responses, and documents outcomes.
- Provides patient and family education, including discharge planning.
- Collaborates with interdisciplinary teams to ensure coordinated, patient-centered care.
- Prioritizes care based on complex physical, psychosocial, and safety needs.
- Delegates tasks, supervises staff, and provides mentoring and feedback.
- Promotes teamwork, communication, and departmental collaboration.
- Participates in unit goals, quality improvement, and patient safety initiatives.
- Ensures compliance with regulatory standards and promotes a safe care environment.
- Supports cost-effective care and applies evidence-based practices to improve patient outcomes.
