Job Description
- Position: Case Manager RN
- Patient Ratio: 1:25-30 (Adults & Geriatrics)
- Schedule: 5x8 hour days (8:30 AM - 5:00 PM, Monday to Friday) plus one full weekend every 4 weeks.
Requirements
- Experience:
- Minimum of 2 years recent acute care hospital case management experience.
- Discharge planning experience required.
- Level 1 Trauma experience preferred.
- Technical Skills:
- Proficient in Microsoft Office Suite, Cerner, CarePort, AllScripts, and Powerchart.
- Must have Epic experience.
- Licensure:
- Active Florida/Compact RN license.
- Education:
- Degree from an accredited nursing school or an institution approved by the Florida Board of Nursing (FBON).
- Must have 2 years of recent acute care experience within the past 24 months in a US hospital.
Additional Information
- Local Candidates: Considered within a 60-mile radius.
- Documentation Requirements:
- I-9 and government-issued photo ID required.
- Various certifications (ACLS, BLS, PALS, NRP, etc.) may be required based on unit description.
- Completion of online orientation and various training modules is mandatory.
- Background check and health documents (including TB test, vaccinations, and physical exam) are required.
Certifications and Competencies
- Certifications:
- ACLS, BLS, PALS, NRP, and others as applicable.
- Competencies:
- Medication Test/Pharmacology Competency Exam.
- Annual skills checklist.
Health and Safety Requirements
- Vaccinations:
- COVID-19 vaccination preferred.
- Seasonal flu vaccination documentation required by December 1st.
- Drug Testing:
- Negative drug screen required within 12 months of start.
References
- Professional References: 2 references required from within the last 2 years.
