Job Opportunity: PT Home Health (Therapy/Rehabilitation)

Location: San Jose, California
Employment Type: Travel/Contract


Position Details

  • Specialty: Physical Therapist (PT) and Speech-Language Pathologist (SLP) in various counties including Sonoma, Marin, Contra Costa, Alameda, Santa Clara, San Joaquin/Stanilaus, and Sacramento.
  • Patient Load: Focus on patients per week rather than per day. Minimum 23-24 visits per week required to qualify for a 40-hour guarantee.
  • Types of Cases: Home Health, Wound Care, Geriatric Home Health Patients.
  • Contract Length: 4-6 months (less experienced clinicians must commit to 6 months, non-negotiable).

Experience Requirements

  • Years of Experience: Minimum 2 years overall as a clinician, with at least 1 year in Home Health.
  • Prior Travel Experience: Not required; first-time travelers are accepted.

Work Schedule & Requirements

  • Weekend Requirement: No weekends required.
  • On-Call: Not required (discretionary but not mandatory).
  • Territory: May include surrounding counties; details discussed during interview.
  • Locals Accepted: Yes, within a 50-mile radius.
  • Mileage Reimbursement: IRS standard from first patient to last patient (not to/from home).

Certifications & Licenses

  • Required at Submission:
    • Valid State License (must have in hand; no pending licenses accepted).
    • BLS (Basic Life Support).
  • Preferred/Additional Certifications:
    • ACLS (Advanced Cardiac Life Support).
    • PALS (Pediatric Advanced Life Support).

Compliance & Documentation

  • All required documents must be completed and available before starting patient care. Compliance processing may take 2-3 weeks.
  • Required documents include but are not limited to:
    • Government-issued photo ID (Driver’s License/State ID).
    • Background checks (within 1 year, including SS Trace, County searches, Nationwide, Sex Offender).
    • Emergency Contact Form.
    • Physical Exam (within 6 months, signed by MD/DO, free of active communicable diseases).
    • Immunizations and Titers:
      • Tetanus/Diphtheria/Tdap (within 10 years or exemption).
      • Negative TB test or Chest X-Ray (within 1 year).
      • Measles, Mumps, Rubella (proof of immunity or vaccination as per birth year guidelines).
      • Hepatitis B (titer, declination, or vaccine series).
      • Varicella (titer, vaccination, history of chickenpox, or declination).
    • Drug Testing: 9-panel drug screen within 1 year.
    • Seasonal Flu Vaccination or Declination (required during flu season).
    • COVID Vaccination or approved medical/religious exemption.
    • Auto Insurance Card (if applicable).

Work Environment & Other Details

  • Dress Code: Any color scrubs, closed-toe shoes.
  • Charting: Must be completed onsite using Kinnser/Wellsky; no charting allowed at home.
  • Parking: Free residential parking available.
  • COVID & Flu Policy: COVID vaccination required with religious exemption allowed; flu vaccination can be declined with proper declination form.
  • References:
    • 2 professional references required for travel, allied, per diem acute care.
    • 1 professional reference for per diem non-acute care.
