Job Opportunity: PTA Home Health (Therapy/Rehabilitation)

Job Description

• Provide home health physical therapy assistant services including home visits and periodic office meetings.
• Average patients per day: 5-6
• Types of cases: Start of Care, Follow Ups
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM
• No weekend rotation or on-call required
• Must be willing to travel up to 75 miles from home clinic; travel mileage reimbursed at IRS standard rate (patient home to patient home only)
• Use EMR system: Home Care Home Base
• Open position due to workload

Required Qualifications

• Minimum 1 full year of recent Adult Home Health experience (first-timers not accepted)
• Current Physical Therapy Assistant (PTA) license in hand (pending license accepted only if it takes a few days to obtain)
• CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted)
• Reliable transportation and current driver’s license
• Agency-required liability insurance
• Must be willing to cover territory primarily Boone County, KY

Additional Requirements

• Must complete standard background checks including:

  • 7-year work/living history verification
  • Sex offender search
  • EPLS/GSA/SAM and HHS/OIG checks
  • State-specific background checks as applicable
• Drug screening: 10-panel required
• Health documents required prior to start and updated annually:

  • Physical exam within 12 months
  • Negative TB test or chest X-ray within 12 months
  • Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella
  • Flu vaccination or documented declination for current season
  • COVID vaccination series or declination
• Must review and sign various compliance, confidentiality, and policy documents prior to start and annually thereafter
• Two professional references required
• Work history verification for at least last 2 employers or 7 years

Notes

• No guaranteed hours; if patient census is low, contractor may be floated to nearby clinics to pick up hours
• Weekend and on-call requirements vary by location but are not required for this assignment
• Locals accepted at the same rate
• Must be able to meet all state-specific licensing and background requirements for Kentucky

This position is ideal for experienced PTAs with recent home health experience looking for a 13-week travel contract in Kentucky with a stable Monday-Friday schedule and no weekend or on-call duties.
