Job Opportunity: Home Health LVN / LPN

Employment Type: Travel/Contract
Contract Length: 20 weeks

Job Description

• Skilled home care experience required (private duty nursing experience only not accepted)
• Must have home health experience with OASIS training and experience
• IV skills and wound care
• General medical diagnoses including high-risk diagnoses such as:

  • Heart Failure (HF)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Myocardial Infarction (MI)
  • Pneumonia
  • Stroke
• Post-operative care
• Patient care tasks include: cardiopulmonary assessment, basic wound care, ABI compression dressing, wound vac dressing, Pleurx/Aseptdrain care, Foley catheter care, staple/suture removal, glucometer use, pro-time machine use, SQ/IM injections, lab draw venipuncture, IV skills, PICC line or Midline care and dressing change, SQ port access/deaccess
• Home skilled visits: 6-8 per day
• Flexible day/shift hours based on office needs (examples: Mon-Fri 8a-5p or 9a-6p, Thurs-Mon or Fri-Tues shifts, alternating weekends)
• Visits and hours are self-scheduled based on HomeCare office needs

Required Qualifications

• License: Valid LVN / LPN license for Florida
• Experience: Minimum 1 year of experience in home health nursing (higher years of experience not specified)
• Skilled home care experience required (private duty nursing experience only not accepted)
• OASIS training and experience required
• BCLS certification required
• Valid driver’s license and reliable car required for home visits
• COVID-19 vaccination required or approved exemption on file
• Flu shot required or declination on file; mandatory mask wearing at all times while at work if declined

Certifications & Competencies

• BCLS (Basic Life Support) - AHA or RQI accepted
• Medication/Pharmacology competency exam as applicable (LPN/LVN - Long Term Care Med Test)
• Color Blind Testing (Relias Color Blind Exam) required prior to start
• Competency exams required as applicable to profession (score 80% or higher)
• Other certifications may be required based on unit or role (e.g., ACLS, PALS, NRP, EFM) but not specified as mandatory for this role

Health & Safety Requirements

• Physical exam within 6 months of start
• Negative TB test (PPD, Qgold, or Tspot) within 10 weeks of start; chest X-ray and TB questionnaire if prior positive PPD
• Annual TB questionnaire required in lieu of annual TB test
• Immunizations or titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, series, or declination)
  • Varicella
• Standard 9-panel drug test within 30 days of start (instant tests not accepted)
• Seasonal flu vaccination or declination with mask requirement if declined
• COVID-19 vaccination required or approved exemption
• Respirator medical clearance annually if respirator use is required

Additional Requirements

• Background checks including 7-year history, EPLS/GSA/SAM, HHS/OIG
• Education verification required (including foreign equivalency if applicable)
• Fingerprinting: Florida AHCA Level II fingerprint clearance required prior to start
• References: 2 references from past 2 years required
• Employment application and skills checklist required
• Facility-specific documents including photo for badge, personal/auto insurance, dress code & tobacco policy, and various compliance modules must be completed prior to start
• Laptop, MiFi, and portable printers provided for documentation and scheduling

Work Environment

• Home health setting with Medicare, Medicare Advantage, and commercial insurance patients
• Use of BayCare electronic systems for forms, scheduling, and timesheets
• Scrubs: Any solid color scrub
