Physical Exam within 1 year of start
Fit Mask test within 1 year, updated annually (N-95 or Halyard Orange Duckbill masks accepted; PAPR not accepted)
Tetanus/Diphtheria/Tdap vaccine within 10 years
Negative TB test or Chest X-Ray (PPD, QGold, or TSpot within 1 year; Chest X-Ray within 2 years for positive responders)
Rubeola (Measles), Mumps, Rubella: Positive titer or 2 vaccines required; booster if low/equivocal titer
Hepatitis B: Positive titer, series, or declination required
Varicella: Positive titer or 2 vaccines required; booster if low/equivocal titer
Drug Testing: Negative 10-panel drug screen within 30 days of start; plan for completion within 72 hours of contract signing
Flu Seasonal Vaccination or Declination during flu season (Oct-April); mask required if declined
