- Physical exam within 1 year of start

- Negative TB test or chest X-ray within 12 months
- Flu vaccine during flu season or declination form
- COVID-19 vaccine or declination if onsite
- Drug screen (10 panel) within 90 days of start
- Fit mask test (for travel starts on or after 2/5/2024)
- Respirator safety clearance (for travel starts on or after 2/5/2024)
References: Minimum 2 supervisor references covering 1 year of employment within last 3 years (6 months for CNAs)
