- Health Requirements:
- Physical exam within 1 year of start
- Immunizations: TDAP, MMR (Rubeola, Mumps, Rubella), Hepatitis B, Varicella
- Negative TB test or chest X-ray within 12 months
- Flu vaccine during flu season or declination form
- COVID-19 vaccine or declination if onsite
- Drug screen (10 panel) within 90 days of start
- Fit mask test (for travel starts on or after 2/5/2024)
- Respirator safety clearance (for travel starts on or after 2/5/2024)
- References: Minimum 2 supervisor references covering 1 year of employment within last 3 years (6 months for CNAs)
- Employment Application: Must include at least last 5 years of employment with explanations for gaps or overlaps
- Skills Checklists: Required annually for applicable units
- Competency Exams: As applicable by profession/specialty
- State-Specific Checks:
- Washington and Oregon candidates have additional state-specific background and registry checks if applicable
- Alaska background check application and fingerprinting handled by facility
