Job Description

  • Provide skilled home health therapy services including assessment, cardiopulmonary/circulatory care, documentation, and use of equipment/devices such as gait belts, rolling walkers, sit-to-stand lifts, sliding boards, collars, supplemental oxygen, prosthetics, and wheelchair assessments.
  • Manage orthopedic and neurological patients.
  • Develop and implement home exercise programs.
  • Administer medications and provide patient and family education.
  • Recognize signs and symptoms of physical, sexual, and psychological abuse.
  • Focus on safety and injury prevention.
  • Perform billing tasks related to therapy services.

Patient Population:
Med/surg, cardiac, orthopedic (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson’s, failure to thrive, respiratory patients, neurological, tracheostomy, trauma, amputees (AKA, BKA), and activities of daily living (ADLs).

Work Setting:
Skilled home care visits (Medicare, Medicare Advantage, and Commercial insurance).
6-8 home visits per day (minimum 6 points per day, may increase).
1-2 weekends per contract.
Self-scheduled visits and hours based on office needs.


Requirements

  • Experience: Minimum 1 year of home health experience required.
  • Licensure: Must be state licensed in therapy/rehabilitation specialty.
  • Certifications:
    • BLS (Basic Life Support) required.
    • OASIS training preferred.
  • Other:
    • Valid driver’s license and reliable car required for home visits.
    • COVID-19 vaccination required or exemption form on file.
    • Flu shot required or declination on file; mask wearing mandatory at all times while at work.
    • Scrubs: Any solid color scrub.

Documentation and Compliance

  • Background checks including SS trace, county searches, nationwide, sex offender, education verification, EPLS/GSA/SAM, HHS/OIG required within 1 year of start.
  • Foreign equivalency evaluation required if education is outside the USA or Puerto Rico.
  • Fingerprinting: Florida AHCA Level II fingerprint clearance required prior to start; must be current and on file.
  • Education verification required for all healthcare professionals.
  • Physical exam within 6 months of start.
  • Negative TB test or chest X-ray within 10 weeks of start; TB questionnaire annually.
  • Immunizations required: Measles, Mumps, Rubella, Hepatitis B, Varicella (titer or vaccines with boosters as needed).
  • Drug testing: Standard 9-panel within 30 days of start.
  • Seasonal flu vaccination or declination with mask requirement.
  • COVID-19 vaccination or approved exemption.
  • Respirator medical clearance annually if respirator use is required.
  • References: 2 references from the past 2 years required.
  • Employment application and skills checklist required.

Competency Exams and Training

  • Medication/Pharmacology competency exam required (score 80%+).
  • Color blind testing required prior to start.
  • Dysrhythmia/EKG competency exam required for RNs (score 100% on lethal arrhythmia section).
  • Stroke education CEUs required (8 total credits including NIH Stroke Certification and mRS).
  • Crisis Prevention Institute (CPI) certification required for behavioral health and psych ER (may be completed onsite after start).
  • BayCare Online Learning Center modules must be completed prior to start (4-8 hours of training depending on role).

Facility Specific Requirements

  • Photo for badge required prior to start (clear, professional, solid background).
  • Personal or auto insurance required and must be current for home health positions.
  • Compliance with BayCare dress code and tobacco policy.
  • BayCare Health MSP - Fit Mask Acknowledgement required.
  • BayCare Health MSP - AHCA Privacy Policy, Affidavit, and Demographic Information Sheet required for all candidates.
  • BayCare Home Care - Phlebotomy Fact Sheet provided and tested onsite.
  • BayCare Health MSP - EMR Signature Request Form required for home health candidates.

Additional Notes

  • Laptop, MiFi, and portable printers provided for documentation and scheduling.
  • Documentation systems used: BayCare Forms, BayCare Scheduling, Ace Timesheet.
  • Candidates must be prepared for self-scheduling visits and hours.
  • Prior travel experience is not explicitly required for this position.
