Required Qualifications

• Bachelor of Science in Nursing (BSN)
• Current Illinois Registered Professional Nurse license
• American Heart Association Basic Life Support (BLS) certification

Certifications (as applicable)

• BLS (Basic Life Support): Required for all RNs
• ACLS (Advanced Cardiac Life Support): Required for certain specialties (e.g., Cardiac Tele, Cath Lab, ICU, ER, etc.)
• PALS (Pediatric Advanced Life Support): Required for pediatric specialties
• NRP (Neonatal Resuscitation Program): Required for Labor & Delivery, Mother/Baby, NICU, Nursery
• EFM (Electronic Fetal Monitoring): Required for Labor & Delivery (AWHONN certified)
• State License Verification: Primary source verification required within 30 days of start
• NURSYS Verification: Required for all RNs within 30 days of start
• NIH Stroke Certification: Required for ICU specialty types

Background and Compliance Requirements

• 7-year criminal background check within 30 days of start
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
• Education verification of highest degree completed
• Illinois Healthcare Worker Registry eligibility and fingerprinting (must be completed in Illinois)
• State-specific searches and compliance per Illinois regulations

Health and Immunization Requirements

• Physical exam within 1 year of start, confirming good health and no physical limitations
• Fit mask test (3M Aura 1870+) within 12 months, updated annually
• Tetanus/Diphtheria/Tdap vaccine within 10 years (Tdap required, TD not accepted)
• Negative TB test or chest X-ray per protocol, updated annually for clinical patient-facing roles
• Rubeola (Measles), Mumps, Rubella: Positive IgG titer or 2 vaccines with boosters if needed
• Hepatitis B: Completed vaccine series or positive IgG titer; declinations accepted with signed form
• Varicella: Positive IgG titer or 2 vaccines with boosters if needed
• Drug testing: 10-panel drug screen within 30 days of start; no instant or dilute results accepted
• Seasonal Flu vaccination or facility-specific medical/religious exemption required during flu season (Oct 1 - Mar 31)
• COVID-19: Initial vaccine series or declination required

Additional Requirements

• Employment history covering 7 years with explanations for gaps over 30 days
• Annual skills checklist (not required for non-clinical positions)
• Completion of facility-specific training modules and agreements (e.g., confidentiality, information security, cannabis acknowledgment) prior to start
• Completion of Illinois Sexual Harassment Training annually
• OSHA Respiratory Safety clearance required if fit test is failed
• Accommodation request forms for vaccine exemptions or failed fit tests must be submitted promptly
