Job Details
- Shift: Monday to Friday, 8:00 AM - 4:30 PM (start time can flex)
- Schedule: 5 days per week, 40 hours guaranteed
- On-call: Not required
- Weekend work: Not required
- Floating: May float to hospice within scope if needed; no floating to sister facilities
- Setting: Community-based, patient’s home with office on campus
- Average daily census: 26 patients
- Top diagnoses treated: COPD, CHF, orthopedic procedures (including joint replacements), neurological diseases
- Vehicle: Reliable vehicle required
Experience & Qualifications
- Required Years of Experience: Minimum 1 year in Home Health or Orthopedic/Aging (OA) physical therapy
- Epic experience: 1 year preferred
- Prior travel experience: Not explicitly required
- Licensure:
- Alaska Physical Therapist License required (Compact PT license accepted)
- Pending state license not accepted
- Certifications:
- Basic Life Support (BLS) required
- Education:
- Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program (foreign equivalent accepted)
Job Responsibilities
- Evaluate patients and develop treatment plans and goals based on patient records, interviews, and observations
- Provide patient treatment according to plan of care, modifying as needed
- Complete documentation accurately and timely, meeting regulatory and licensure standards
- Collaborate with healthcare team members to ensure coordinated patient care
- May supervise therapy assistants, aides, volunteers, and provide clinical instruction to students
- Participate in quality improvement, new service development, staff meetings, and educational activities
- Perform other duties as assigned

Compliance & Requirements

- Negative TB test or chest X-ray within 1 year
- Measles, Mumps, Rubella (MMR) immunity or declination
- Hepatitis B immunity or declination
- Varicella immunity or declination
- Drug testing within 30 days of start
- Seasonal flu vaccination or approved declination during flu season
- Orientation modules and facility-specific training/tests must be completed prior to start with 100% passing score
- Masking policy requires N95 fit testing and clean-shaven facial hair for mask use
This position is ideal for a Physical Therapist with at least 1 year of home health experience, who is licensed in Alaska and comfortable working in a community/home health setting with a stable Monday-Friday schedule and no on-call or weekend requirements.
