Step Down RN Travel Contract

Location

• Hazard, Kentucky

Contract Details

• 13-week contract
• Local candidates only; must live within 75 miles of the facility
• Extension will not be approved

Schedule

• Night shift
• 12-hour shifts
• 36 hours per week
• Every other weekend required
• Blocked scheduling cannot be accommodated
• Float required within scope of practice

Unit

• Stepdown unit
• Typical ratio: 1:4/5

Required Qualifications

• Associate of Science in Nursing / Associate Degree in Nursing
• Valid RN license in Kentucky
• Stepdown experience within the last year
• 2 years of Stepdown experience
• BLS issued by the American Heart Association
• ACLS issued by the American Heart Association
• NIHSS
• Flu vaccination required; declinations are not accepted
• Driver's License
• Must provide an ARH coversheet with matching address on the driver's license and coversheet

Compliance / Pre-Start Requirements

• Background check
• Education verification
• Drug screen
• 2 references within 1 year of start date
• 7-year work history verification
• Physical exam within 1 year of start date
• Negative TB test or chest X-ray within 1 year of start date
• Tdap vaccination or declination within 1 year of start date
• Measles, mumps, rubella, varicella, and hepatitis B documentation
• Fit mask completion onsite
• Orientation safety manual acknowledgement
• Skills checklist
• Competency exams and core training modules
• State vulnerable adult registry check
• APS/CPS search results
• Employee health attestation and portal documentation

Additional Notes

• Medical and religious flu exemptions may be considered case by case
• Latex allergy accommodations are available, with prior notification required
• PAPRs cannot be accommodated due to facial hair
