Job Overview

• Travel/contract role in Roseburg, Oregon
• Home health setting
• Day shift, 5x8 schedule
• 13-week assignment
• 40 hours per week

Key Responsibilities

• Provides and coordinates home nursing care based on physician orders, agency policies, and the nursing process
• Supervises and evaluates home health aides and LPN/LVNs
• Typical caseload is 5–8 patients per day
• Home visits are required, with periodic office meetings
• Territory coverage includes Roseburg and surrounding areas

Required Qualifications

• Valid RN license in Oregon
• Associate of Science in Nursing / Associate Degree in Nursing
• Home Health experience within the last year
• 1 year of Home Health experience
• Hospice experience
• One completed travel assignment
• BLS issued by American Heart Association or American Red Cross
• Driver's License
• Experience with Homecare Homebase
• Experience with OASIS

Schedule and Work Expectations

• Weekend schedule varies by location and profession
• Potential for rotating on-call coverage
• Standard schedule is typically Monday through Friday, 8:00 AM to 5:00 PM
• Some positions may use a Baylor shift schedule of Friday through Monday, 4x10s

Charting and Documentation

• Must be proficient with HomeCare HomeBase and have used it within the last 6 months
• OASIS proficiency is required for RNs

Additional Notes

• Local candidates are accepted at the same rate
• Guaranteed hours are not provided
• Scrubs are required
• Mileage is reimbursed at the IRS standard rate
