Physical Therapist Assistant - Home Health

Job Description

• Provide home health physical therapy assistant services with a focus on clinical excellence and compassionate care.
• Caseload ratio: 1:1 treatments, approximately 7-10 visits per day depending on patient locations.
• Work exclusively in the Home Health setting; no floating to other departments.
• Schedule options: Monday through Friday or Tuesday through Saturday (no Sunday visits preferred).
• Guaranteed 40 hours per week, day shift (8:00 AM to 4:00 PM).

Required Qualifications

• Valid Physical Therapy Assistant license in Tennessee.
• Minimum of 1 year of Physical Therapy Assistant experience within the last year.
• Experience in a Home Health setting.

Additional Information

• No weekend requirement, but open to a Tuesday through Saturday schedule.
• Locals are accepted; pending licenses are not accepted.
• No on-call duties.
• No floating outside of home health scope.
• Orientation includes fingerprinting and fit mask testing onsite.
• Badge photo required prior to start (passport-style, plain background).
• Must complete facility-specific forms including job description acknowledgment, code of conduct agreement, demographic information, and COVID-19 vaccine exemption if applicable.
• Physical exam within the last 12 months required but not annually updated.

Shift Details

• Day shift, 5 days a week, 8 hours per day (40 hours/week).
• Typical hours: 8:00 AM to 4:00 PM.
