Schedule

• Night shift: 4 days per week, 9 hours per day (22:30 to 8:00)
• 36 hours per week
• Every other weekend required

Required Qualifications

• Valid Radiology Technologist license in Illinois
• Radiology Technologist with at least 1 year of experience, including experience within the last year
• X-Ray Technician experience
• American Registry of Radiologic Technologists (ARRT) Radiography (R) certification
• Illinois Emergency Management Agency (IEMA) Accreditation
• Basic Life Support (BLS) certification (AHA only, RQI accepted)

Required Documentation & Compliance

• Current and valid Driver’s License or Government Photo ID with address preferred
• Proof of name change if legal name differs from compliance documents (marriage license or divorce decree)
• 7-year criminal background check within 30 days of start, including SSN trace, aliases, county, national, federal, sex offender, OIG, SAM, FACIS 3, and education verification
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
• Education verification of highest level completed (diploma/transcript must match degree)
• Primary source verification of state license within 30 days of start
• Color blind testing exam with passing score required
• Competency exam with passing score required
• Physical exam within 1 year of start confirming good health and no physical limitations
• Fit mask test (3M Aura 1870+) within 12 months of start and annually thereafter
• Tetanus/Diphtheria/Tdap vaccine within 10 years (TD not accepted)
• Negative TB test or chest x-ray within required timeframes; annual updates required for clinical patient-facing roles
• Immunity or vaccination proof for Rubeola (Measles), Mumps, Rubella, Varicella, and Hepatitis B (declinations accepted for Hep B)
• Drug screening (10 panel) within 30 days of start; no instant or dilute results accepted
• Seasonal flu vaccination or facility-specific medical/religious exemption required during flu season (Oct 1 - Mar 31)
• COVID vaccination series or declination required
• OSHA Respiratory Safety clearance required if fit test failed
• Illinois Healthcare Worker Registry fingerprinting and eligibility verification required prior to start (must be completed in Illinois)

Additional Requirements

• Must complete facility-specific training modules and sign confidentiality, privacy, information security, cannabis acknowledgment, and policy acknowledgment forms prior to start
• Sexual harassment training required annually with wet signature on certificate
• Accommodation request forms required for vaccine or fit test exemptions, with timely submission for review
• Employment history covering 7 years with explanations for gaps over 30 days
• Skills checklist required annually
