Job Opportunity: LVN / LPN - Long Term Care

Schedule
- Shift options: 4x12 hour shifts or 3x12 hour shifts (traveler's choice)
- Shifts are variable but caregivers are assigned either day or night shifts (no rotation except for urgent needs)
- Guaranteed 60 hours per 2 weeks; may be called off for one 12-hour shift per 2 weeks
- Must be willing to work at least 2 of the 3 major winter holidays (Thanksgiving, Christmas, New Year's); preference for availability on all holidays

Requirements / Qualifications
- Valid LPN / LVN license in Alaska
- Minimum 1 year of LPN / LVN experience within the last year
- Experience in a Long Term Care setting
- One completed travel assignment
- Basic Life Support (BLS) certification issued by the American Heart Association
Licensure and Certifications
- Alaska Food Worker Certification required; obtained onsite during orientation (one-time $10 fee)
Additional Requirements
- Candidates must live at least 50 miles from the facility (locals not accepted)
- No more than 2 weeks of requested time off (RTO) allowed for a 26-week contract; all RTO requests must be submitted with application and approved before offer or contract signing
- Floating within scope of practice may be required to meet facility needs
- Block scheduling accommodations are not available

Vehicle Use
- Facility provides a vehicle for use during assignment (available 72 hours before start and must be returned within 24 hours after end)
- Only the traveler may operate the vehicle
- Traveler must provide valid driver’s license and proof of insurance, including rental car coverage
- Vehicle use limited to within 30-mile radius and “around town” only; no off-road use
- Vehicle must be returned clean and with a full tank of gas
- Tire chains available at traveler’s expense
- Vehicle must be returned during any time off and reissued upon return
Required Documentation and Compliance
- Government-issued photo ID (Driver’s License/State ID)
- Social Security Card (as applicable)
- Background check within 90 days covering all locations lived/worked in past 7 years; must be updated annually
- Education verification for nursing degree
- NURSYS license verification
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work
- Immunizations and health requirements including:
- Negative TB test or chest X-ray within 12 months
- Tetanus/Diphtheria/Tdap vaccine within 10 years (declination accepted)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination (Alaska placements require positive titer and vaccine series)
- Varicella vaccination or positive titer
- Seasonal flu vaccine during flu season (declination accepted)
- COVID vaccination record preferred (declinations not required)
- Drug screening: 9-panel drug screen within 12 months (instant acceptable)
- Respirator medical clearance required annually for travel candidates
- Annual completion of regulatory and specialty-specific training modules via LMS (Workramp)
- References: Minimum 2 references covering 1 year of employment in a comparable LTC specialty/setting within last 3 years
- Employment application covering at least last 5 years of employment with explanations for gaps or overlaps
Other Notes
- On-call is not required
- First-time travelers are not accepted
- No pending license acceptance
- Locals (within 50 miles) are not accepted
- Floating policy acknowledgment required; may be asked to float to nearby facilities within reasonable distance
