Registered Nurse - Home Health

Location: Monroe, Georgia

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Average patient visits: 5-8 per day (assigned 5-10 patients daily).
• Types of visits include Start of Care, Wound Care, and Follow-up Visits.
• Work primarily involves home visits with periodic office meetings.
• Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (standard 5x8 schedule; some locations may have 4x10 shifts Friday-Monday).
• On-call requirement: Yes, approximately 1 weekend per month.
• Must be willing to travel up to 75 miles from the home clinic.

Required Qualifications

• Minimum 1 full year of recent Adult Home Health or Hospice nursing experience (depending on specialty).
• First-time travelers are not accepted; recent specialty experience is mandatory.
• Current Registered Nurse (RN) license for Georgia required (license must be in hand; pending licenses not accepted).
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Proficiency with HomeCare HomeBase (HCHB) EMR system within the last 6 months required; retraining required if last use was over 6 months ago.
• RNs must also be proficient in OASIS documentation.

Additional Requirements & Compliance

• Must have a valid driver’s license and personal or auto insurance policy under the candidate’s name or list the candidate as a driver.
• Background checks including 7-year history, sex offender, drug screening (10-panel), and state-specific clearances are required.
• Physical exam within 12 months confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination or declination documentation required.
• COVID-19 vaccination series or declination required.
• Education verification and work history verification (at least 2 most recent employers or last 7 years) required.
• Must complete various compliance documents and training prior to start, including confidentiality, HIPAA, privacy policies, and home health/hospice-specific policy acknowledgments and quizzes.

Work Environment & Expectations

• Dress code: Scrubs.
• Documentation and charting done electronically using HomeCare HomeBase, OASIS, and Outlook.
• Productivity expectations: approximately 30 units per week.
• Local candidates accepted under the same terms as travelers.

Travel & Mileage

• Travel up to 75 miles radius from home clinic required.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not reimbursed).

Notes

• Contract may lead to permanent hire, but this is primarily a contract assignment.
