Physical Therapy Acute Care Travel/Contract Position
Location: Lafayette, Indiana
Specialty: Therapy/Rehabilitation (Physical Therapy Acute Care)
Facility Type: Short-Term Acute Care Hospital
Shift: Monday to Friday, 5 days of 8-hour shifts; at least one weekend per month required
Contract Length: 13 weeks
Requirements
- Experience: Minimum 1 year of relevant experience
- Licensure: Indiana state license required and must be in hand before starting (no pending licenses accepted)
- Certifications:
- BLS (Basic Life Support) - AHA only, RQI acceptable
- ACLS, PALS, NRP, Chemotherapy, TNCC, NIH Stroke Certification, and others may be required depending on cost center/unit
- Holiday Work: Must work 2 out of 3 major holidays
- Guaranteed Hours:
- 5x8 hour shifts = 40 hours/week
- 3x12 hour shifts = 36 hours/week
- 4x12 hour shifts = 48 hours/week
Facility & Work Environment
- Beds: Approximately 731 staffed beds across units
- Charting System: Cerner
- Dress Code: Royal Blue scrubs, black shoes
- Parking: Free
Required Documentation & Compliance
- Valid Driver’s License or Passport (clear copy)
- Physical exam completed within 12 months of start date
- Fit mask test within 12 months (specific approved respirators required)
- Immunizations and Titers:
- Tetanus/Diphtheria/Tdap (strongly recommended)
- Negative TB test or chest X-ray within 1 year
- Rubeola (Measles), Mumps, Rubella, Varicella: Positive titer or 2 vaccines required; no declinations accepted
- Hepatitis B series, titer, or signed declination required
- Seasonal Flu vaccination/documentation required during flu season (no declinations accepted)
- Drug Testing: Negative 10-panel drug screen within 12 months
- Background Check: Within 90 days of start, including 7-year history and education verification
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
- References: Minimum 2 from recent employment with appropriate supervisory titles
- Skills Checklist: Completed within 12 months and annually thereafter
- Education Verification and Transcript/Diploma: Official copies required; may be pending at start if needed
- Competency Exams: Medication test/pharmacology and unit competency exams required
- Color Blind Testing: Ishihara exam or equivalent with 100% minimum score
Additional Notes
- Locals are accepted for this travel/contract position
- Must complete facility-specific forms and orientation packets prior to start
- HIPAA and confidentiality agreements required
- On-site fit testing available only as a last resort and not at this facility
- PAPR use requires prior facility approval
