Physical Therapist - Outpatient

Location: Bend, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Outpatient oncology rehabilitation setting
  • Caseload primarily includes cancer patients, neurologic, and geriatric patients
  • Schedule options: 4x10-hour shifts or 5x8-hour shifts (Day shift: 8:00 AM - 2:30 PM)
  • Average daily patient census: 9
  • Clinician may float within scope to meet facility needs, including other facility locations
  • Locals not accepted unless residing within a 75-mile radius (proof of address required)
  • Pending license accepted (can be obtained within 48 hours)
  • No on-call required
  • Clinicians must wear their own scrubs; no required color

Required Qualifications

  • Valid Physical Therapist license in Oregon
  • Minimum 1 year of Physical Therapist experience within the last year
  • BLS certification required

Background and Credentialing Requirements

  • 7-year criminal background check within 90 days of start, updated annually, including:
    • SSN trace
    • Counties lived/worked/school
    • FACIS L3
    • DOJ sex offender search
    • Nationwide check
    • Education verification (highest level)
  • EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
  • Diploma or transcript required and must be on file
  • Primary source verification of state license required
  • Photo for badge and multiple facility-specific agreements must be completed within 48 hours of signing and no later than 2 Wednesdays prior to start

Health and Safety Requirements

  • Physical exam within 1 year of start
  • TDAP vaccine within 10 years or declination accepted
  • Negative QGold TB test within 1 year; if positive, negative chest X-ray within 5 years and annual TB questionnaire required
  • Annual TB questionnaire required for all candidates
  • Proof of immunity or vaccination (or declination) for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B (positive titer required; vaccine series alone not accepted)
    • Varicella
  • Negative 12-panel drug screen within 90 days of start (not required annually)
  • Seasonal flu vaccination or agency declination/exemption required during flu season (ends 3/31)
  • COVID vaccines preferred but not required; vaccination record or declination/exemption required
  • Respiratory safety clearance required annually

Competency and Skills

  • Medication test/pharmacology competency exam as applicable
  • Annual skills checklist required

Additional Notes

  • Maximum of 7 total days off allowed during the 13-week assignment
  • Prior travel experience is not required for this position
