Medical Social Worker - Travel/Contract

Location: Worcester, Massachusetts
Contract Length: 13 weeks

Job Summary

• Conduct eligibility screenings, comprehensive intake, and follow-up assessments to determine client eligibility and inform treatment planning.
• Perform client evaluations including bio-psychosocial assessments, diagnostic formulation, and initial treatment plan recommendations.
• Provide individual counseling to assist clients in achieving treatment goals.
• Develop and document treatment plans with behaviorally based goals and objectives; maintain progress notes.
• Refer clients to internal and external resources.
• Participate in multidisciplinary team meetings and case discussions; seek consultation as needed.
• Engage in utilization management and assist with accreditation processes.
• Build relationships with agencies to support client recovery and facilitate referrals and care coordination.
• Collaborate closely with clinical team members (Recovery Coach, Nurse, Psychiatrist) to coordinate client services.

Work Schedule & Requirements

• Shift Options:

  • Day shift (5 days x 8 hours): 8:00 AM – 4:30 PM
  • Day shift (4 days x 10 hours): 8:00 AM – 6:30 PM
• Guaranteed 40 hours per week.
• Flexibility required for weekend work as needed.
• Must be willing to float to all locations within a 35-mile radius.
• Ability to travel and transport patients using personal vehicle may be required.
• Advanced notice will be provided for daily assignment locations.

Required Qualifications

• Valid Social Worker license in Massachusetts (LMFT, LCSW/LICSW, LMHC, or PsyD).
• Master's degree in a relevant field.
• Minimum of 1 year of recent experience as a Social Worker, Licensed Marriage and Family Therapist, or Licensed Professional Clinical Counselor.

Compliance & Documentation Requirements

• Background Check:

  • 7-year criminal background check within 30 days of start, including SSN trace, county searches, national criminal search, sex offender search, and federal exclusions (OIG, SAM).
  • Background check must be updated annually.
• Education Verification: Third-party verification of highest completed education required; transcripts acceptable if verification is unperformable (diplomas not accepted).
• Physical Exam: Required within 12 months of start, including physician statement of fitness to work without restrictions, signed by MD, DO, NP, or PA.
• Drug Testing: Required within 30 days of start; includes multiple drug panels. Instant drug screens acceptable.
• Immunizations/Vaccinations or Declinations (declinations not accepted for Pediatrics, Maternity, NICU):

  • COVID-19 (most recent vaccine or facility-specific declination)
  • Flu seasonal vaccination or declination (required during flu season)
  • MMR (Measles, Mumps, Rubella)
  • Varicella
  • Hepatitis B (3-dose series, positive titer, or declination)
  • Tetanus/Diphtheria/Tdap
• TB Testing:

  • Negative TB test or chest X-ray within required timeframes.
  • TB questionnaire required annually or if positive TB history.
• References: Two references within three years of start.
• Employment Application: Must include 7 years of employment history with explanations for gaps over 30 days.
• Skills Checklist: Required prior to start and updated annually as applicable.

Additional Notes

• Preference given to candidates with no return-to-office (RTO) days, but up to 5 RTO days allowed.
• Must be able to start on May 4th.
• Flexibility with hours and locations is essential.
• Candidates must complete required competency exams and attestations as applicable.

This position requires a dedicated social services professional comfortable with travel, flexible scheduling, and comprehensive client care coordination within a community health setting.
