Outpatient Physical Therapist - Travel/Contract

Location: Fond du Lac, Wisconsin
Contract Length: 13 weeks

Job Details

• Shift: 4 days per week, 10-hour shifts (7:30 AM - 6:00 PM)
• Daily Caseload: 10-12 patients
• Start Date: ASAP
• Weekend Requirement: No
• On-call Requirement: No
• Floating: No
• Local Candidates: Not accepted (must be outside 70-mile radius)
• First-time Travelers: Accepted

Requirements/Qualifications

• Valid Physical Therapist license in Wisconsin

Licensure & Certifications

• License: Wisconsin Physical Therapy license required (pending license accepted upon submission)
• Certifications: Basic Life Support (BLS) required

Special Requirements

• Must shave for mask fit testing to ensure proper seal (no exceptions; failure to shave will result in cancellation)
• Fit testing will be completed onsite during the first week of orientation
• PAPRs and CAPRs are not provided and personal use is not allowed

Facility & Work Environment

• Adult Trauma Level III facility
• 120 staffed beds
• Charting system: EPIC

Compliance & Documentation Requirements

• Criminal background check within 12 months required
• Physical exam within 1 year indicating good health required
• Negative drug screen within 12 months required
• Immunizations and health screenings including TB test, flu vaccine (or approved exemption), and others per facility policy
• OSHA Respirator Clearance required prior to start
• Completion of facility-specific orientation modules and checklists required
• Various state-specific background and caregiver searches required for Wisconsin

Additional Notes

• One RTO (request time off) allowed per contract
• Multiple post-start orientation checklists depending on assignment type
• Candidates must complete all required compliance and facility-specific documents prior to or during orientation
