Job Details
- Specialty: Therapy/Rehabilitation
- Shift: Monday to Friday, 8:30 AM – 5:00 PM
- Work Setting: Home Health
- Local Radius Rule: No locals accepted within 75 miles
- State License: South Carolina PTA license required prior to starting (pending license accepted if must be obtained before start)
- Experience: Must have Home Health experience
- Attendance Policy: More than two missed shifts without reasonable explanation during the 13-week contract may result in contract review and possible cancellation
Required Certifications & Licenses
- South Carolina PTA license (must be active prior to start)
- BLS (Basic Life Support) certification (AHA required, RQI acceptable)
- ACLS, PALS, NRP, TNCC, EFM certifications as applicable (check specific unit requirements)
- Orientation Safety Manual Acknowledgement
- State License Primary Source Verification
Required Background & Compliance Checks
- 7-year criminal background check within 1 year of start, updated annually
- South Carolina Statewide Search required regardless of residence/work history
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and updated for renewals
- Education verification (highest level of education must be associates degree or higher; certificates alone are not sufficient)
- Drug testing within 60 days of start (10 panel plus Oxycodone and Marijuana)
- Physical exam within 1 year of start, updated annually
- Fit mask (N95) within 1 year of start, updated annually
- Negative TB test or chest X-ray within 3 months of start
- Immunizations or proof of immunity for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
- Flu vaccination or declination documentation for flu season (Nov 1 – Mar 31)
- COVID vaccination (Dose 1 and 2 required or declination form)
Required Documentation
- Proof of I9 or attestation
- Government issued photo ID (must be carried during hours of service)
- Social Security Card (if used on I9)
- Employment application
- Skills checklist updated annually (if applicable)
- References: 2 references within 1 year of start for home health settings
- Facility-specific photo for badge (headshot on plain background)
Requirements/Qualifications
- Valid Physical Therapy Assistant license in South Carolina
- Minimum of 1 year of experience as a Physical Therapy Assistant
- Physical Therapy Assistant experience within the last year
- BLS (Basic Life Support) certification
- Experience in a Home Health setting
Additional Notes
- All Return to Office (RTO) requests must be included in the initial submission; late requests will not be considered
- Guaranteed hours are based on hours signed in the contract
- Candidates must comply with all facility-specific policies and testing requirements
- Pass/fail testing (e.g., medication calculation, cardiac arrhythmia) required for some roles but not for PTA Home Health
