Physical Therapist (PT) - Home Health

Location: Pittsfield, Massachusetts
Employment Type: Temporary
Contract Length: 13 weeks

Job Overview

• Provide physical therapy services to home health clients.
• Deliver direct patient care and collaborate with the care team.
• Participate in developing comprehensive patient care plans.
• Use rehabilitation equipment such as splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities.
• Must be Home Health Certified.

Requirements

• Experience:

  • Minimum 2 years of combined acute nursing care (hospital-based) and home care experience.
  • Acute care experience preferred.
  • Certified home health experience with proficiency in OASIS is required and non-negotiable.
• Licenses/Certifications:

  • Valid Massachusetts PT license.
  • BLS (AHA) certification.
• Other:

  • Valid driver’s license required.
  • Ability to work independently with minimal supervision.
  • Must provide at least two specific interview dates/times (no "anytime" availability accepted).
  • Cannot reside within 50 miles of the facility.
• Profile Submission Requirements:

  • Updated work history.
  • Skills checklist relevant to the position (current within 1 year).
  • One supervisory signed reference from within the past two years.
  • All licenses and certifications must be uploaded with the profile.

Additional Notes

• Non-billable orientation: 8 hours.
• No call, call-back, or charge responsibilities.
• Requested time off during the assignment must be noted upfront; no more than 5 days of time off allowed.

This position is ideal for a physical therapist with strong home health certification and experience, who can work independently and meet specific interview scheduling requirements.
