Job Description

• Provide and document nursing care for sick and injured patients in their homes.
• Promote, maintain, or restore health, prevent illness, and support rehabilitation.
• Collaborate with healthcare providers and community representatives to coordinate home care.
• Evaluate patient and family responses to nursing interventions.
• Function independently under general guidance, following unit-specific policies and protocols.
• Utilize critical thinking, time management, communication, planning, delegation, and problem-solving skills.
• Maintain emotional stability in emergency situations.
• Accept on-call responsibilities and be flexible with work hours.
• Ability to drive in inclement weather is required.
• Maintain confidentiality and adhere to patient rights and customer service standards.
• Must reside more than 50 miles from the facility to be considered.

Required Qualifications

• Valid LPN / LVN license in Massachusetts
• LPN / LVN (1 year of experience)
• LPN / LVN experience within the last year
• BLS issued by American Heart Association or American Red Cross
• Experience in a(n) Home Health setting
• Driver's License

Additional Information

• Non-billable orientation: 8 hours.
• Requests for time off during assignment limited to no more than 5 days and must be submitted upfront.
• Must comply with all regulatory and reimbursement guidelines related to home care.

This position requires a licensed practical nurse with home health experience who can work independently, manage a flexible schedule, and provide high-quality care in a community setting.
