Job Opportunity: X-Ray Technologist
Location: Jamaica Plain, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Schedule: Monday to Friday, 12 PM - 8 PM
Contract Length: 13 weeks
Required Credentials and Compliance for Onboarding
Certifications
- BLS (Basic Life Support): Required for onboarding. Acceptable certifications include AHA, Red Cross, CHSF, or Military. Preference for AHA certification.
Licenses
- Professional License Verification: Must provide primary source verification of professional license/certification within 30 days of start date. MA Conditional Licenses are not accepted.
Personal Health Requirements
- N95 Fit Test: Required within 365 days of start date and updated annually. Must be completed on the official MGB N95 Respirator Fit Test Record Form. No religious exemptions.
- MMR Vaccination or Immunity: Documentation of two measles, two mumps, and one rubella vaccine, or two MMR vaccines, or proof of immunity via blood test.
- Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed. Pharmacy or provider-administered flu mist accepted; self-administered not accepted.
- TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date, updated if there is a break >6 months.
- Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date, updated if break >6 months.
- Respirator Medical Clearance (RMC) Attestation: Required if fit test is required; must be completed prior to fit testing.
- Hepatitis B: Strongly preferred but optional. Documentation of vaccination series or immunity proof or declination.
- Tdap Vaccine: Preferred within last 10 years but optional.
- Positive PPD with Negative Chest X-Ray: Required if applicable (optional otherwise).
- TB Symptom Survey: Optional if positive PPD; must confirm no symptoms of TB infection.
Professional Requirements
- Criminal Background Check: Must be completed within 45 days prior to start and updated if break >6 months. Includes county and international searches if applicable.
- Massachusetts CORI Check: Must be completed within 30 days prior to start and updated if break >6 months.
- National Sex Offender Check: Completed within 30 days prior to start and updated annually.
- Exclusion Checks: Completed within 30 days prior to start and annually. Includes OIG, GSA, MassHealth, and NH Medicaid exclusion lists.
- Social Security Number Trace: Must be run with criminal check covering last 7 years.
- Education Verification: Verified through background check company; must verify highest level of education achieved.
- References: Two recent references required, at least one supervisor or above in the relevant skillset.
- Resume: Must include past 7 years of work experience with city and state for all positions and education.
- Skills Checklist: Required for submission.
- Candidate Submission Coversheet: Required for submission.
- Confirmation of Assignment/POI Form: Required for all initial assignments and extensions, updated if start date changes.
