Registered Dietitian

Location: Stockton, California
Employment Type: Clinical-Allied
Contract Length: 26 weeks (approximately 6 months)
Shift: Monday to Friday, 8:00 AM – 4:30 PM onsite
Hours: 40 hours per week (8 hours per day)

Job Summary

The Registered Dietitian is responsible for developing and implementing a nutrition services program in collaboration with the Interdisciplinary Team (IDT). Key duties include conducting nutrition assessments, delivering nutrition counseling, overseeing menu planning and meal delivery, coordinating supplements, and supporting care transitions. The role requires compliance with all relevant regulations and active participation in team meetings and quality improvement activities.

Key Responsibilities

• Conduct nutrition assessments to determine participant needs, preferences, and goals.
• Participate in IDT meetings to develop and update participant care plans.
• Deliver and document nutrition interventions, including special diets and counseling for participants and families.
• Oversee menu planning and meal delivery to ensure safety, regulatory compliance, and correct special diets.
• Coordinate supplement orders and distribution.
• Provide training to day center staff on meal delivery programs as needed.
• Collaborate with physicians and IDT members to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home).
• Support intake and enrollment processes for prospective participants.
• Monitor and report changes in participants’ condition or psychosocial status to the IDT.
• Complete all required documentation and mandated trainings timely and accurately.
• Maintain confidentiality and protect participant and team information.
• Follow organizational policies and participate in quality improvement and staff training activities.
• Demonstrate flexibility to meet evolving business needs.

Required Qualifications

• Valid Registered Dietitian Nutritionist (RDN) license in California
• Registered Dietitian Nutritionist (RDN) credential with a minimum of 4 years of experience
• Recent experience (within the last year) as a Registered Dietitian Nutritionist
• Basic Life Support (BLS) certification
• Experience working with geriatric patients

Preferred Qualifications

• Bachelor’s Degree (preferred but not required).

• Willingness and ability to work varied schedules, including evenings and weekends if needed.
