Required Qualifications

Valid RN license in Texas
Acute Care Case Management (1 year of experience)
Acute Care Case Management experience within the last year
BLS
Associate of Science in Nursing / Associate Degree in Nursing

Preferred

Familiarity with Meditech and Careport
CCM

Work Environment

Day shift staffing includes 1 RN and 1 SW
Patient ratio: 1:20–30
Floating: varied CM assignment
