Physical Therapist - Home Health

Location: Elizabeth, West Virginia
Setting: Home Health (patients seen in home health setting)
Contract Length: 13 weeks

Job Details

• Patient Load: Approximately 5-6 units per day

  • Types of visits include Start of Care (SOC), Discharge, Recertification, and Regular visits
• Common Patient Diagnoses:

  • General orthopedics (hips, knees)
  • Generalized weakness
  • Strengthening
  • Cardiac rehabilitation
• Coverage Area:

  • 8 counties in West Virginia (including Braxton, Gilmer, Calhoun, Nicholas, Webster, Clay, Randolph, Pocahontas)
  • Scheduling and patient assignments may be adjusted based on therapist location to optimize mileage and productivity
• Scheduling:

  • Self-scheduling allowed
  • Flexibility to move patients within schedule as long as initial and reassessment guidelines are met
• Shifts: Monday through Friday, no weekend or holiday requirements

Required Qualifications

• Licensed Physical Therapist eligible to practice in West Virginia
• BLS (Basic Life Support) Certification
• Must meet any additional PT licensing requirements for the state of West Virginia
• Positive attitude toward learning new systems (e.g., Epic documentation system)
• Experience with general PT duties in home health setting preferred

Orientation

• Department-specific orientation conducted at the local office
• Focus on Epic documentation training tailored to clinician needs
• Orientation expected to occur promptly, especially for travel contracts

Additional Information

• Dress code: Navy scrubs or khaki bottoms with navy polo
• Rehab support staff includes PT Assistants (PTAs)
• No prior travel experience explicitly required for this position
