Social Worker – Care Coordinator (Discharge Planning)
Location: Jackson, Mississippi
Contract Length: 13 weeks
Job Summary
Seeking experienced Social Worker Case Managers (LMSW or LCSW) for a Level 1 Trauma center. Role focuses on inpatient/acute care case management and discharge planning in a large medical center setting.
Required Qualifications
- Valid Case Management license in Mississippi
- Minimum 2 years of case management experience, including inpatient settings
- Case management experience within the last year
- Experience with Epic EMR system
- BLS Certification
Work Schedule & Environment
- 12-hour day shifts, 36 hours per week
- Rotating schedule: 7a-7p every Wednesday & Thursday and every other Friday, Saturday, Sunday
- No weekend requirement outside of scheduled shifts
- Holiday shifts rotated as needed
- Case load: 15-20 patients/consults per day
- Dress code: Any color scrubs or business professional attire
- Department orientation: 2 weeks including general facility orientation
- Will work alongside case management team; no solo work
Additional Information
- Fingerprint background check required; remote process takes 10 days or in-person at local campuses the week prior to start
- 50-mile radius rule applies for travelers
- Collaboration with Respiratory Therapy and Pharmacy for discharge planning as needed
- Facility handles complex patient placements with limited financial assistance criteria for uninsured or underinsured patients
Interview Process
- Virtual interview via Webex (20-30 minutes) within 72 business hours of application
- Candidates must provide specific availability for on-camera interview
This position is ideal for highly experienced social workers with strong acute inpatient case management and discharge planning skills looking to work in a fast-paced, large Level 1 trauma center environment.
