Job Description
- Position: Radiologic Technologist working weekend night shifts
Required Credentials & Compliance for Onboarding
Professional Credentials
- MGB Badge Photo: Color photo taken within past 6 months; no selfies, no filters, plain background, symmetrical headshot, no eyeglasses
- BLS Certification: AHA, Red Cross, CHSF, or Military accepted (AHA preferred)
- ARRT Certification
- License/Certificate Verification: Primary source verification of professional license/certification within 30 days of start date; MA Conditional Licenses not accepted
- MGB Criminal Background Check: Within 45 days prior to start date; includes county and international searches if applicable
- MGB Massachusetts CORI: Criminal history check for MA within 30 days prior to start date
- MGB Resume: Past 7 years of work experience with city & state included (required for submission)
- MGB References: Two recent references, at least one supervisor or above, in relevant skillset (required for submission)
- MGB Skills Checklist: Required for submission
- MGB Confirmation of Assignment/POI Form: Required for all initial assignments and extensions
- MGB Education Verification: Verification of highest education level achieved
- MGB SSN Trace: Must be run with criminal check, listing addresses/aliases for past 7 years
- MGB National Sex Offender Check: Ran within 30 days of start and updated annually
- MGB Exclusion Checks: Ran within 30 days of start and annually; includes OIG, GSA, MassHealth & NH Medicaid exclusion lists
Personal Health History & Testing
- Fit Test (N95 Respirator): Within 365 days of start date, updated annually; must be completed on MGB form; no religious exemptions
- MMR Vaccination or Immunity: Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test
- Influenza Vaccine: Required during flu season (Sept 15 - April 30); no exemptions allowed
- TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date; updated if >6 month break
- Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date; updated if >6 month break
- Respirator Medical Clearance (RMC) Attestation: Required if fit test is required; completed prior to fit test
- Positive PPD - Negative Chest X-Ray: If positive TB test, chest x-ray documentation showing no active TB required (optional)
- Hepatitis B: Strongly preferred; record of 3 vaccines or proof of immunity or declination (optional)
- Tdap Vaccine: Preferred within last 10 years (optional)
- MGB TB Symptom Survey (if Positive PPD): Optional annual screening form to confirm no TB symptoms
