Job Description

• Provide direct nursing care following established policies, procedures, and protocols.
• Implement and monitor patient care plans.
• Monitor, record, and communicate patient condition appropriately.
• Coordinate all disciplines for well-coordinated patient care.
• Follow physician and nursing orders.
• Assess and coordinate patient discharge planning with healthcare team members.
• Requires critical thinking, decisive judgment, and ability to work with minimal supervision.
• Must be able to work in a stressful environment.

Required Certifications and Licenses

• ACLS (American Heart Association)
• BLS (American Heart Association)
• Nursys verification within 30 days prior to start (including FACIS Questionnaire if disciplinary actions exist)
• Current nursing license with primary source verification within 30 days prior to start
• Ascension Specialty Exam (minimum 80% passing score, updated every 3 years)
• Ascension Color Vision Test (updated every 3 years)
• Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly)
• NIH Certification (Group A yearly, Groups B-F every 2 years)
• Ascension EKG Exam (minimum 80% passing score, updated as per latest edition)
• Ascension Onboarding Requirements Education Attestation
• Ascension Nursing Contract Worker Onboarding (completed onsite)
• Ascension Nova StatStrip (pre-hire modules for non-allied positions)
• Ascension Epic Test-Out (test results and training checklist with signed attestation)

Health and Immunization Requirements

• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; updated annually
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within one year, and completed TB questionnaire; updated annually
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within specified timeframes prior to start
• Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Ascension 12 Panel Drug Screen: Within 30 days prior to start; includes testing for 12 specified drugs; must include MRO review if applicable

Professional Requirements

• Driver's License/State ID: Must be current, match address on file, and comply with compact license rules if applicable
• Ascension Profile: Resume or professional profile required
• Two Professional References: At least two qualifying references prior to first assignment
• Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender; completed within 30 days of first assignment and updated annually
• EPLS (Excluded Parties List System) Check: Run within 90 days of start; rerun if break in service > 90 days
• Ascension Skills Checklist: Clinical skills checklist completed within one year of start; updated annually
• Education Verification: Verification of education relevant to clinical work within 90 days of start; rerun if break in service > 90 days
• Confirmation of Assignment: Provided upon initial assignment and updated with each extension
• Employment Verification: Third-party verification for past seven years; updated annually or if break in service > 90 days
