Job Overview

• Primarily outpatient physical therapy with occasional float to acute care.
• Responsible for patient-focused care including assessment, planning, intervention, and evaluation following physical therapy standards and physician orders.
• Delegates tasks to Physical Therapist Assistants and Aides.
• Develops and updates individualized treatment plans and goals based on patient assessments.
• Educates patients, families, and caregivers on conditions, prognosis, and treatment rationale.
• Provides supervision to assistants, aides, home health aides, and students per regulatory standards.
• Recommends and assists in securing adaptive/supportive equipment for patients.
• Documents patient responses, treatment changes, and completes billing documentation in compliance with policies and regulatory agencies.
• Functions as case manager for assigned cases.
• Participates in interdisciplinary team meetings and promotes physical therapy services within the community.
• Demonstrates effective communication and teamwork to maximize quality of patient care.
• Promotes education, development, and process improvement among staff and students.

Required Qualifications

• License: Physical Therapist (PT) license [REQUIRED]
• Education: Bachelor’s Degree [REQUIRED]
• Certifications: Basic Life Support (BLS) certification highly preferred within 1 month of hire or transfer
• Successful completion of facility-specific orientation and new hire competency.
• Must be fingerprinted (employer covers cost).

Experience

• No explicit years of experience stated; however, this is a Physical Therapist I level position.
• Prior travel experience is not required.

Physical and Mental Requirements

• Medium physical demand level.
• Ability to occasionally lift/carry 21-50 lbs, frequently lift/carry 20 lbs, and constantly lift/carry 10 lbs.
• Frequent sitting, standing, walking, and postural changes such as stooping, kneeling, or crouching.
• Exposure to communicable diseases and bloodborne pathogens; must follow infection control protocols including use of personal protective equipment.
• Ability to move freely about the facility with or without assistive devices.
• Good vision and hearing (with or without assistive devices).
• Ability to handle multiple tasks, work independently, and maintain regular attendance.
• Required to demonstrate and teach treatment routines to professional and non-professional individuals.
