Care Coordinator

Overview

• Travel/Contract role in La Jolla, California
• Night shift: 3x12 hours, 19:00–7:30
• 36 hours per week
• Contract length: 13 weeks

Role Summary

• Screens patients in the assigned caseload to identify medically vulnerable patients and those who may need care at another location or through contracted programs/vendors.
• Coordinates with the health care team and the patient/family to address issues affecting care and to develop a transition plan of care.

Required Qualifications

• Valid RN license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Case Management (2 years of experience)
• Case Management experience within the last year
• One completed travel assignment
• BLS issued by American Heart Association or RQI
• Experience with Epic

Preferred Qualifications

• ICU or PCU experience preferred
• MS or ED experience may be accepted if clinically strong

Additional Notes

• BLS for Health Care Provider is required as needed for direct patient contact and department need
• BLS is not required within Central Transfer Center
