Registered Nurse I, Home Health

Location: Conyers, GA (travel up to 75 miles from the home clinic) Contract Length: 13 weeks Positions Needed: 1 Setting: Home health (visits in both patients' homes and care facilities)

Schedule

• Day shift, 5x8 hours (8:00 AM to 5:00 PM), Monday to Friday, 40 hours per week
• Some western-territory locations use 4x10 schedules, Friday to Monday
• No guaranteed hours. If census is low, contact the account manager about floating to a nearby clinic for additional hours.
• On call: yes, 1 weekend per month
• Weekend and on-call requirements vary by location and profession and will be discussed during the interview.

Responsibilities

• Provide home visits and attend periodic office meetings
• Average daily caseload of 5 to 8 patients; contractors typically see 5 to 10 patients per day
• Weekly productivity expectation of 30 points. Visit types, documentation, and travel carry different point values.
• Case types: start of care, wound care, follow-up visits, and other adult home health or hospice services
• Client is open to hiring the contractor at the end of the contract.

Requirements

• Associate of Science in Nursing / Associate Degree in Nursing
• Valid RN license in Georgia
• Active license in hand (pending licenses are not accepted)
• Minimum of one full year of recent adult home health or hospice experience, depending on specialty
• Home Health experience within the last year
• Experience working with adult patients
• Completed travel assignment (one completed travel assignment required; first-time travelers are not accepted)
• BLS certification
• CPR certification
• Reliable transportation, a current driver's license, and agency-required liability insurance
• Experience with HomeCare HomeBase (HCHB), used within the past six months. Clinicians with prior HCHB experience outside that window may be considered but must complete retraining during compliance.
• Experience with OASIS (RNs, PTs, and OTs must be proficient in OASIS)
