Location: Studio City, California Employment Type: Travel/Contract Contract Length: 13 weeks

Schedule

• Day shift, 5 x 8-hour days, 8:00 AM to 4:30 PM
• 40 hours per week
• Weekly work guarantee: 36 hours

Assignment Details

• Occupational therapy (OT) in a skilled nursing facility (SNF)
• May float to another nearby location if needed

Required Qualifications

• Valid Occupational Therapist license in California
• Experience in a Skilled Nursing setting
• Basic Life Support (BLS) from the American Heart Association or American Red Cross, current and clear copy

Pre-Start and Compliance Requirements

• Background checks: Standard background check (7-year search, including county, nationwide, and sex offender searches), EPLS/GSA/SAM check, and HHS/OIG check. Background check is updated annually; EPLS/GSA/SAM and HHS/OIG must be done annually.
• Identification: Government-issued photo ID (driver's license or state ID)
• Emergency contact form: Required before start
• Physical exam: Within 1 year of start
• TB: Negative PPD, QuantiFERON Gold, or T-SPOT within 1 year of start. If past positive PPD, a negative chest X-ray dated after the positive and within 1 year of employment is required. Annual TB questionnaire required.
• Immunizations/titers: Rubeola (measles), mumps, rubella, hepatitis B, and varicella documentation (titer, vaccine series, or declination, per the requirements for each). Candidates born before 1957 are subject to client policy.
• Drug screen: 9-panel drug screen within 1 year of start (instant drug screen acceptable)
• Flu vaccination: Vaccine or declination required during flu season (10/1 to 3/31)
• References: 2 professional references
• Skills checklist: Required annually
