Medical Surgical Registered Nurse - Travel/Contract

Location: Gallipolis, Ohio
Contract Length: 13 weeks

Position Details

• Shift: Nights, 3 shifts per week, 12 hours each (7:00 PM – 7:30 AM)
• Patient Ratio: 1:5, flexible up to 1:6
• Charting System: Paragon
• Weekend Schedule: Rotating weekends; no on-call required
• Floating: May be required per facility policy
• Travel Radius: 50 miles
• First Time Traveler: Yes, open to first-time travel nurses
• Dress Code: Any solid color scrub combination

Required Qualifications

• Valid RN license in Ohio
• Minimum 1 year of Medical Surgical nursing experience within the last year
• BLS (Basic Life Support) issued by the American Heart Association
• ACLS (Advanced Cardiac Life Support)
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Compliance & Documentation Requirements

• Background Checks:

  • 7-year criminal background check including SSN trace, counties lived/worked, statewide, national criminal, sex offender, and FACIS Level 3
  • EPLS/GSA/SAM and HHS/OIG checks, updated monthly
• Fingerprinting: BCI and FBI fingerprints within 12 months of start, updated every 5 years; results must be on file within 30 days of start
• Photo ID: Valid government-issued photo ID required at all times on assignment
• Social Security Card: Required
• Physical Exam: Must clear candidate to work without restrictions; completed prior to start
• Fit Mask: Fit testing within 6 months of start with approved mask types (3M 1860 or 3M VFlex 1804)
• Drug Testing: Negative 12-panel drug screen within 30 days of start
• Immunizations & Health:

  • Tdap within 10 years
  • Negative TB test (Quantiferon Gold within 30 days or PPD per unit requirements)
  • Measles, Mumps, Rubella: Positive titer or vaccination series with booster or declination if titers negative/equivocal
  • Hepatitis B: Positive titer required or signed declination
  • Varicella: Positive titer or vaccination series with booster or declination
  • Flu vaccine required for the 2025-26 season or signed declination by 12/31/2025 (for starts before 12/1/2025)
  • COVID vaccination or signed declination required

Orientation & Facility Requirements

• Completion of facility-specific documents prior to start, including:

  • Confidentiality agreements
  • Authorization to release information
  • Contract employee information form
  • Non-employee orientation packet and post-test
  • Age-specific and cultural competency training and post-tests
  • Dress and appearance acknowledgment
  • Commitments to care and code of conduct acknowledgments
  • N95 beard waiver if applicable (only if notified)
  • Signed job description
