Job Opportunity: Licensed Practical Nurse (LPN)
Location

Work Schedule
- Day shift, Monday to Friday (5x8 hours)
- Hours: 8:00 AM to 4:30 PM
- No weekend shifts required
Unit Description
- Hospital-based outpatient clinic providing outpatient services and inpatient consults
Required Qualifications
- Valid LPN/LVN license in Massachusetts
- Minimum 2 years of LPN/LVN experience, including experience within the last year
- At least one completed travel nursing assignment
- Basic Life Support (BLS) certification issued by the American Heart Association
- Experience in Ambulatory Care
Compliance and Documentation Requirements
- Valid government-issued ID
- Massachusetts Statewide iCORI clearance within 30 days prior to start
- 7-year criminal background check within 30 days of start, including SSN trace, county and national searches, and education verification
- Education verification of highest completed education (transcript acceptable if verification unavailable)
- Drug test within 30 days of start (specific drug panels required)
- Physical exam within 12 months of start, including physician statement of fitness to work without restrictions
- Immunizations or valid declinations for:
- COVID-19 (most recent vaccine or facility-specific declination)
- Influenza (seasonal vaccine or declination required during flu season)
- Tetanus/Diphtheria/Tdap (declinations not accepted for Pediatrics, NICU, Maternity)
- Measles, Mumps, Rubella (positive titer, vaccines, or declination; declinations not accepted for Pediatrics, Maternity, NICU)
- Hepatitis B (positive titer, 3-dose series, or declination with documentation)
- Varicella (positive titer, vaccines, documented history, or declination)
- Tuberculosis screening:
- Two PPD tests (one within 90 days, one within 1 year) or QuantiFERON/T-Spot within 90 days
- If positive, chest X-ray within 5 years and facility-specific TB questionnaire required
- Medication and Telemetry exams (pass/fail testing) required on first day of orientation with minimum 80% score; only one attempt allowed
- NURSYS license verification within 30 days prior to start
- Massachusetts Nurse Aide Registry Search (tracking only)
Additional Requirements
- Two professional references within the last three years
- Employment application covering 7 years of employment history with explanations for gaps over 30 days
- Skills checklist completed prior to start and updated annually as applicable
Notes
- Candidates must be able to start on June 24 without exceptions
- Compliance documents and certifications must be current and uploaded as required
- Failure to pass required competency exams on orientation day may result in immediate cancellation of assignment
