Metropolitan area with a heavily populated homeless population (no direct work with this population, but must be comfortable navigating the area)

Employment Type: Travel/Contract
Contract Length: 13 weeks
Schedule: Part-time, 3 shifts per week, 8 hours per shift (Day shift, approx. 8:00 AM to 5:00 PM)
Guaranteed Hours: 20 hours per week
Average Patients per Day: Variable
Types of Cases: Home Health focusing on gait, strength, range of motion
Charting System: Kinnser/Wellsky
Dress Code: Scrubs, any color
Mileage Reimbursement: IRS standard; Uber rides covered as alternative

Valid Speech Language Pathologist license in California
Minimum 1 year of Speech Language Pathologist experience within the last year
Minimum 1 year of experience in a Home Health setting, including OASIS experience
Basic Life Support (BLS) certification

Government-issued photo ID (Driver’s License or State ID)
Emergency Contact Form required prior to start
Background Check within 1 year, including SS Trace, County searches (last 7 years), Nationwide, Sex Offender registry; updated annually
EPLS/GSA/SAM and HHS/OIG checks must be done annually
Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year (with specific protocols for past positives)
Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella as applicable
9 Panel Drug Screen within 1 year of start (instant drug screen acceptable)
Seasonal Flu Vaccination or Declination required during flu season (Oct 1 - Mar 31)
