Job Description

Seeking a Physical Therapist with acute care experience to work in an acute care hospital setting. This is a travel contract position.

Requirements/Qualifications

• Valid Physical Therapist license in Wisconsin
• Minimum 1 year of Physical Therapist experience within the last year
• One completed travel assignment
• Experience in an Acute Care setting

Required Certifications

• Wisconsin Physical Therapy License (preferred prior to start)
• ACLS (Advanced Cardiac Life Support) – ARC or AHA acceptable
• BLS (Basic Life Support) – ARC or AHA acceptable
• Other certifications as applicable to the profession

Background and Health Requirements

• 7-year background check including SS trace, county searches for last 7 years, nationwide sex offender check
• Physical exam within 1 year of start
• Negative TB test (QGold) within 1 year of start; chest X-ray within 60 days if applicable
• Immunizations or proof of immunity for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Negative 10-panel drug screen with expanded opiates within 14 days before start (instant tests not accepted)
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (October to March)

Additional Requirements

• Photo for badge (passport style, shoulders up, no filters)
• Compliance checklist and confidentiality statement to be completed prior to start
• References: 2 professional references verified within 2 years of start
• Annual skills checklist as per agency standards

Notes

• Prior travel experience is not required for this position
• This role is in an acute care hospital environment requiring acute care PT experience
• Candidates must be prepared to comply with all health and background screening requirements before starting
