Job Opportunity: Pediatric Physical Therapist (Travel/Contract)

Work Schedule

• 4 days per week, 9-hour shifts (4x9) with every 4th weekend off (Saturday & Sunday)
• Alternative schedules considered: 5x8 or 4x10
• Shift start no earlier than 7:30 AM, shift end no later than 6:00 PM
• No on-call required

Requirements

• Minimum 1 year of experience as a Physical Therapist
• Minnesota license required (can submit application without having license in hand; temporary license issued quickly once documents are received)
• Must commit to applying for MN license once offered the position
• Basic Life Support (BLS) certification required
• Accepts first-time travelers
• Must live at least 25 miles away from the facility (no local candidates accepted)
• Experience with EPIC charting preferred
• Scrub color: Royal Blue

Background Checks & Compliance

• Criminal background check required within 1 year of start, including:

  • 7-year search
  • Social Security trace
  • County checks
  • Sex offender registry
  • Nationwide check
  • Education verification (not required for CNAs)
  • International checks if candidate has lived/worked outside the US
• GSA/SAM and OIG excluded provider checks required
• Fingerprinting via Minnesota DHS Net Study mandatory before start (no starts with fingerprints pending)
• Third-party education verification required (diploma acceptable with proof of verification attempts)
• Physical exam within 1 year of start
• Drug testing (9 panel) within 1 year, not required annually
• Two references from recent employers within 2 years required
• Skills checklist and work history verification as applicable

Certifications

• BLS (Basic Life Support) required
• ACLS, PALS, NRP, TNCC, EFM certifications as applicable to role (not specifically required for this PT role)
• State license primary source verification required

Health & Immunization Requirements

• Tdap vaccination required (no religious declinations accepted)
• Negative TB test or chest x-ray per MN state guidelines (2-step PPD or equivalent within 90 days)
• MMR (Measles, Mumps, Rubella) proof of immunity or vaccination required (time sensitive)
• Varicella immunity or vaccination required (time sensitive)
• Hepatitis B vaccination, positive titer, or declination accepted
• Seasonal flu vaccination or declination required annually (mask required if declined)
• COVID-19 vaccine preferred; medical/religious exemptions accepted
• COVID-19 booster required only for long-term care candidates with specific testing and masking protocols

Facility Specific Information

• Education modules (~16 hours) completed during orientation (except Long Prairie location where modules must be completed prior to start)
• Dress code must be reviewed prior to start; PTs wear royal blue scrubs
• Photo for badge required (grey background, no selfies, JPG format)
• Confidentiality agreement must be signed prior to start
• Badge must be returned on last day of assignment

Additional Notes

• Candidate must complete Minnesota DHS Net Study Fingerprint Questionnaire immediately after signing contract
• Fingerprinting must be completed within 2 weeks of registration
• Candidates may need to travel to a livescan location if not within driving distance
• Background checks and fingerprinting requirements are strictly enforced by Minnesota law
